Anesthesia Coverage Needs: Monday - Friday; 5- 8’s ONLY; 7a-3p -No Call, No weekends. - MUST BE AVAILABLE TO WORK 20 SHIFTS A MONTH
Rotating between surgery center 1-2 days a week, Hospital 3-4 days a week.

Operating Rooms: Southside-15 St. Augustine-4, St. Vincent 8, St. John 8, Riverside 23
Primary Site for Recruitment (50 Mile Rule)-Surgery Center (109Y)-We will not likely float them beyond the facilities that are on the St Augustine side of town. About a 20 minute drive
Cross credential: Ascension, St. Vincent Clay County, Riverside, Southside, St. Johns Couty
Regional 1099 Pool-Not offered at this time
Care team model : It's always at a 1:3 or 1:4 ratio (never higher than 4).
None have Trauma designation
At any time will the clinician split a shift between hospitals in 1 day or will it just be one day at one hospital and once day at the surgery center? Not for the SAASC – the distance is generally too far for a split shift. If they’re working at one of our other hospitals, some have ASCs nearby that may allow for some split shifts from time to time.
This is a new building.
Qualifications
Must be more than 50 miles from main site
Must have Active FL License
Must have open availability
CV, EPA, Skills Checklist must be presented with presentation
ACLA, BLS Required
Availability: 1st possible date they would be available?
How many shifts would they be able for per month?
What month is their schedule completely open?
Any shift preferences (Day, Night, Blocks…etc)?
Any dates in the next 3-6 months they can’t work?
Any Holidays in the next 3-6 months they would be open to work. ?
If provider does not match to all requirements on the Job Order, you must request permission from MSP Coordinator prior to submitting a Name Clear submission.
The attached ICA must be used for any provider that is approved for credentialing. This version is also in Trio under your Name & then by clicking Help. Please do not utilize any past versions.

MUST have 2 years of experience and be able to provide 24 months of case logs
