Cross Credentialing: Provider will rotate St Lukes Baptist, Baptist Medical Center and North East Baptist.
Credentialing timeframe: 90 days
Start: ASAP after credentialing
Length of contract: 1 year
Cases: Ortho, Urology, General, GI, GYN
Shift: 2x12+2x8. Can also do 4x10. Day off per week per scheduler's needs. No set days.
GWW: 44 hours
Medical Direction
Practice will screen candidates.

MUST fill Submission Questionnaire and attach to presentation file
MUST double check and report malpractice history
